The Medicare rebate for patients referred by their GP under an Enhanced Primary Care Plan is $47.85 per consultation.

Aged and Disability Pensioners:

  • Bulk-Billing is available for Aged and Disability Pensioners referred under an Enhanced Primary Care Plan scheme, so there are no out of pocket expenses.

Non-Pensioners:

  • Non-pensioners who are referred by their GP under an Enhanced Primary Care Plan are required to pay the gap (the difference of the consultation fee and the Medicare rebate).
What do I need to bring?

If you require a Medicare rebate, you must bring 2 forms completed by your doctor. They are:

  • a GPMP (General Practitioner Management Plan) form, otherwise known as a ‘721 form’
  • a TCA (Team Care Arrangement) form, otherwise known as a ‘723 form’

These forms should contain information relating to your medication regimen and your current treatment plan. Please note that you cannot receive a Medicare Rebate until we have received these documents.
