We can design exercise therapy programs for home or for a local facility of your choice. Alternatively, many of our clients attend our Strength Clinic.

Our exercise therapy and lifestyle consultations are aimed at improving the quality of life for people with chronic disease and complex illness. Conclusive evidence shows that exercise works like medicine and is beneficial in the management of a variety of conditions including:

We help you sustain long term changes to your physical activity and lifestyle habits. Our consultations are based on the principles of self-management, incorporating motivational interviewing, working in collaboration with you to design the optimal program, and building your confidence through goal-setting for progressive achievements.

You may require 2 to 5 exercise therapy and lifestyle consultations depending on your current physical activity levels and functional capacity. For example, if you currently have a sedentary lifestyle, an effective treatment model may require up to 5 consultations over a 12 week period. 

This ensures that activity levels are progressively increased and physical status is improved optimally. Additionally, you will acquire the necessary self-management skills for long-term changes in your lifestyle. You may attend the clinic for a follow-up consultation every 3 to 6 months thereafter.

*Medicare rebates are only available for patients referred under a Chronic Disease Management Plan by their GP.

  • Generous rebates also available from most Private Health Funds.
  • Rebates available from Department of Veterans Affairs (DVA).
What do I need to bring?

If you require a Medicare rebate, you must bring 2 forms completed by your doctor. They are:

  • a GPMP (General Practitioner Management Plan) form, otherwise known as a ‘721 form’
  • a TCA (Team Care Arrangement) form, otherwise known as a ‘723 form’.

These forms should contain information relating to your medication regimen and your current treatment plan. Please note that you cannot receive a Medicare Rebate until we have received these documents.
