Patient Privacy
The Lifestyle Clinic is bound by the regulations of the Commonwealth Privacy Act 1988 and the amendments released in December 2001. In order to provide services that our clients require, we record personal information, such as:
- name
- date of birth
- address
- telephone numbers
- medical and personal information
- referral information
- bank account and credit card details
The Lifestyle Clinic understands the sensitivity of these details and will only access and use personal information for the purpose of providing Clinic services. Personal information is collected either from the client themselves or via referral letters from third party sources. Therefore the accuracy of the information provided is dependent upon the source of the information, making regular updates necessary. All personal information is stored on a secure database as well as in a secured hard copy format. Only authorized staff will have access to the information.
Clients have the right to access any personal details, inspect their records, take notes or make a hard copy of the information that we hold on file about them. Clients can change their contact information at any time by contacting the Lifestyle Clinic. Proof of identity will be required prior to granting access to files or for the purpose of providing new or updated information for client records.
If a client believes there has been a breach of privacy, he/she should contact the Lifestyle Clinic Manager and provide proof of identity. A written confirmation of the complaint may be requested. Every endeavour will be made to handle complaints quickly.
